Persistent Severe Itching, Red Patches, Bumps: What's Wrong?

I started itching about 3 weeks ago. The itching comes and goes but is always present. I have an itching fit at least once or twice a day. Sometimes I will notice red blotchy patches and other times my skin is just reddish in color with tiny red bumps everywhere. The best way I can explain is what your skin looks like after getting out of a very hot bath. I'm tearing my skin up from scratching. My head even goes through itchy patches. What is wrong with me please? I seriously am starting to feel like it's possible to scratch yourself to death.
