Stomach pain in the back when lying down facing up can be a result of stomach gas/bloating, positional discomfort or less commonly a disease of the pancreas and surrounding organs.
If you feel pain when you lie on your back and your abdomen stretches, it may just be due to gas or bloating in the stomach or muscular pain from being stretched. Less commonly, it may be due to a diseases of the pancreas and gallbladder that lead to inflammation and pain when they are compressed as you lie down facing upwards.
